Registration for Fall Soccer 2010

Registration for Fall Soccer will be held on Monday April 12th 6 - 8 p.m., Tuesday April 13th 6 - 8 p.m., and Saturday April 17th 10 a.m. - 12 p.m. at the Conrad Weiser Middle School. Please see the requirements for registration on the flyer sent home with your children, or the link provided below. For U9 and above players it is CRITICAL that you provide the proper paperwork in order for us to complete your registration... If you do not know your child's age group, please use the chart below. Volunteers will be on hand at registration to help you through this process...

Fall 2010 / Spring 2011 Age Groups:

Age Groups are based on date of birth in relation to the start of a Fall Season, or August 1st of that year. If your child is born before August 1st, look for their birth year in the 3rd (far right) column to determine their age group. If born after August 1st, look in the 2nd (middle) column. Spring Soccer is considered to be a part of the preceding Fall Season.

**** IMPORTANT ****

• We are not accepting incomplete registration data/payments.
• A $10 late fee will apply to anyone who registers after April 17th.
• If you do not pay when you register online, check or cash must be provided before your leave registration night. We will not accept unpaid registration forms; before you are given a computer to register, you must confirm that you intend to pay online on site with cash or check. If you want to register but pay later, you will not be allowed to register online, and late fees will be applied to any registrations after the last registration night.
• Please provide ALL contact information on your registration form, including alternate emails from each parent, cell phones, and any related contact information that will keep all parents informed.
• [!!] DON'T FORGET TO ORDER A UNIFORM [!!]

U9 and Above only:

• If you come to registration without a 1x1 in. photo for his/her pass you will be required to purchase a digital photo for $5.
• If you are a new player that does not have a birth certificate on file with our club you must have a copy of their birth certificate when you come to registration, or you will be sent home to go get it.
• If you need to order a copy from Harrisburg or out of state, do not register until you have it.
• If your child is born outside the US (even a US military base overseas), additional paperwork is required. Make sure you have a legal copy of their birth certificate. and fill out the RG6 and P-12 forms below....
• If your child has played with another club (that is, has a Player Pass) we still need a copy of the birth certificate OR the old player pass in order to confirm date of birth to the League.
• If your child intends to play on a premier team, playing secondary for CW, please let us know at registration.
